Consent to the Processing of Personal Data

I hereby freely, by my own will and in my own interest, in accordance with Federal Law No. 152-FZ dated 27.07.2006 "On Personal Data", give my specific, informed, conscious, unequivocal, and unconditional consent to the processing of all my personal data (including biometric personal data) by Limited Liability Company "Visiology" (LLC "Visiology"), Primary State Registration Number (OGRN): 1157746032400, Taxpayer Identification Number (INN): 7705352195, Registration Reason Code (KPP): 772701001, registered address: 117218, Moscow, ul. Krzhizhanovskogo, d.29, korp.2, mezzanine 3, room Kh, offices 11–17, and/or by third parties, including representatives of LLC "Visiology", provided by me to LLC "Visiology" when entering into and executing contracts and/or agreements (hereinafter referred to as the “Agreement”) and while using the website https://visiology.com (hereinafter referred to as the “Website”), or through social media/messengers (VKontakte, Odnoklassniki, Facebook, WhatsApp, Viber, Telegram, etc.). The data processing includes collection, recording, systematization, accumulation, storage, clarification (updating, modification), retrieval, use, transfer (distribution, provision, access), depersonalization, blocking, deletion, and destruction of personal data, with or without the use of automation tools — that is, all actions listed in paragraph 3, part 1, article 3 of the Federal Law "On Personal Data" — for the purposes of fulfilling the Agreement, using the Website, and sending marketing communications.

Regarding marketing communications, I give my consent to receive information via the following methods:

• SMS messages
• Push notifications
• Email
• Telecommunication networks, including telephone, fax, and mobile communications
• Messages on social networks (VKontakte, Odnoklassniki, Facebook, WhatsApp, Viber, Telegram)

The subject of advertising includes products, works, and services of LLC "Visiology".

I hereby guarantee that the phone numbers I provide on the Website or otherwise to LLC "Visiology" belong to me. In the event I cease to use any of these phone numbers, I undertake to notify LLC "Visiology" accordingly. If I provide a phone number belonging to a third party on the Website, I guarantee that I have obtained consent from that third party to receive promotional messages from LLC "Visiology" in accordance with this Consent.

This consent is given for the period from the moment of entering into the Agreement or starting to use the Website (whichever comes first) until the moment it is withdrawn in writing.

I accept full responsibility for the accuracy and relevance of the information I provide.

I reserve the right to withdraw this consent by submitting a corresponding written notice, which may be sent by registered mail with delivery confirmation to the address of LLC "Visiology" or delivered in person against a signed receipt from an authorized representative of LLC "Visiology".
